Phase 9 — Files (Task Specs)
Purpose
Phase 9 delivers file management end to end: S3-compatible storage wired through presigned upload/download, a worker file pipeline (virus scan → metadata → thumbnails), file blocks and previews in the editor, plus versions, permissions, quotas, trash, and the security guarantees (malware blocking, MIME sniffing, path sanitization) the platform's security model requires.
Phase Exit Criteria (from ROADMAP.md § Phase 9)
- [ ] S3-compatible storage integration + presigned upload/download
- [ ] File pipeline: virus scan → metadata → thumbnails
- [ ] Previews (images, PDF, video/audio) + file block UI
- [ ] File metadata, versions, permissions, trash
- [ ] Exit: upload of 500 MB file works (multipart); malware sample blocked; preview for each supported kind E2E-tested.

Phase 9 Notes — recorded decisions & conflicts (AGENTS.md rule 29)
The authoritative record is docs/architecture/storage.md § 0
(Implementation Notes).
-
Shared private buckets + key-prefix isolation (not per-workspace buckets, not bucket ACLs):
factory-files+factory-thumbs, bothPrivate; tenant isolation is thefiles/{workspaceId}/…prefix plus server-side permission checks. The raw key is never exposed to clients. -
SigV4 canonical-header set fixed (reference-verified against boto3): presigned requests sign
host(always) + extra signed headers (content-typefor PUT);x-amz-dateis theX-Amz-Datequery param and is never inX-Amz-SignedHeaders. The hand-rolledS3ClientCRUD signer also injectshost. (The scaffold omittedhostand wrongly signedx-amz-date→ 400/403 on every S3 round trip.) -
⚠️ Deviation from Requirement 3 (upload response headers): the response returns
headers: { "Content-Type": <validated mime> }only — noContent-Length. Rationale: the signer does not content-length-lock (spec: "when the S3 SDK supports it" — it does not), and echoing the declared size makes strict HTTP clients refuse a shorter PUT, which is exactly the under-PUT attack the 422UPLOAD_INCOMPLETEcompletion check exists to catch (the integration suite must be able to make that PUT and observe 422 server-side). The declared size is still enforced at completion via HEAD. -
⚠️ Spec-vs-S3 conflict (wrong Content-Type): Requirement 4's letter routes the wrong-CT case through completion as 422
UPLOAD_INCOMPLETE, but the signed URL pins thecontent-type, so S3 rejects the PUT first (403) and completion reports 410UPLOAD_EXPIRED(object never lands). The security property holds (no READY row, no object); the exact code isUPLOAD_EXPIRED, asserted in the suite (test C). -
file.download_deniedaudit is asserted in TASK-0101 although the spec files it under TASK-0104 (the permission-check stub needs the audit to prove the denial path); TASK-0104 builds the full file-permission resolution on top of it. -
⚠️ Pre-existing baselines repaired by this phase's work (were red at the Phase-9 start commit, not caused by Phase 9): (a)
scripts/minio-init.tsdouble-.headersbug —sign()already returns the header object, so bootstrap sent anonymous requests and its HEAD-based existence check mis-read anonymous 403 on a missing bucket as "exists" (buckets were never actually created; data loss when MinIO storage was reset). Now ListAllMyBuckets-based + privacy-verified. (b) 8tests/integration/*.integration.test.tsimported../helpers/…(broken path) instead of./helpers/…. (c)helpers/api.tslacked the globalapi/v1prefix, theAllExceptionsFilter(so specs saw Nest's default error shape), and theRATE_LIMIT_*env overrides — one shared loopback IP self-throttled the suite (auth default 10/min → 429 storms across 20 spec files). (d)@nestjs/corewas undeclared intests/integrationpackage.json. -
⚠️ Pre-existing red NOT in Phase 9 scope (recorded, not fixed — other phases' work, failing at the Phase-9 start commit and unmodified here): API
tsc --noEmit85 errors (Prisma 5.22 model drift in Phase 2–8 code — this phase's files are 0-error), workertsc43,turbo lintred at@factory/web, and the remainingtests/integrationfailures inauth/blocks/comments/databases/pages/permissions/webhooks/workspace(unbuilt routes — e.g.POST /workspaces/:id/permissions404s — and a pre-existingPageService.updateDI/authorization bug). Verified by per-test diff against a stashed-HEAD full run: Phase 9 changes turn 27 failing tests green (74→101 pass) with zero pass→fail regressions. -
TASK-0102 (worker pipeline) recorded decisions & conflicts:
FileVersionupsert is owned by the WORKER, not the API. Requirement 6 says the version row is "created by TASK-0101 on complete; here only verify + backfill." In practice TASK-0101'scomplete()does NOT write aFileVersion(it has nocreatedByto authoritatively set and no stored key beyond the row); the worker's finalize step upsertsFileVersion {fileId, version, key, size, createdBy: file.uploadedBy}on READY. This is the single source of truth and is what the integration suite asserts (a READY row ⇒ a version row; a VIRUS_DETECTED/FAILED row ⇒ none).file.uploadedis emitted TWICE end-to-end through the real API — once atcomplete()(row → SCANNING, TASK-0101) and once by the worker on a fresh SCANNING→READY transition. The worker emits it exactly once (only on the READY transition, never on a no-op re-run). The search indexer upserts idempotently by file id, so the double-emit is harmless; it is recorded here rather than "fixed" because the architecture doc's event contract does not forbid it and the TASK-0101 complete-event is the earlier phase's contract. (The worker-only integration spec asserts the worker's single emit.)File.thumbnailUrl: worker mints, API must re-mint. Requirement 5 wantsthumbnailUrl= a signed 15-min GET "regenerated on read, never stored long-lived." The worker's finalize now mints and stores a 15-min signed GET of the 480 artifact at READY (so the acceptance "thumbnailUrl returns 200" holds immediately after the pipeline; the integration suite fetches it and checks the PNG signature). But a stored URL expires, so TASK-0103'sGET /api/v1/files/:idmust RE-MINT fromthumbnailKeywhen the stored URL is null/expired (the thumbnails bucket is already a presigned-GET source per@factory/storage). Recorded as a TASK-0103 follow-up, not silently dropped.clamdscanrequirement implemented as the identicalzCONTSCANwire protocol (unix socket) — noclamdscanbinary dependency.clamdscanis a thin CLI over the same LOCAL protocol; speaking it directly (thescanFileViaSocketpath) keeps the worker free of a host binary and is byte-for-byte the same contract. TCPzINSTREAMis the alternate transport.mupdfREJECTED (AGPL) for PDF rasterization →pdfjs-dist(Apache-2.0) +canvas(MIT) +sharp(Apache-2.0). See the runbook's dependency table (rule 17).fluent-ffmpegwas not needed —ffprobe/ffmpegare called directly.- Worker S3
x-amz-datesigning bugs (two, found by this task). The hand-rolled workerS3Clienthad the same latent defect on both signing paths (neither was exercised against a real S3 in TASK-0101 — only the storage package's presigner was): (a) CRUD (header-signed):signedRequestaddedx-amz-dateto the outgoing headers after computingSignedHeaders, so the declared set ≠ the signed set →SignatureDoesNotMatch403 on everyputObject/headObject/downloadTo. Fixed by addingx-amz-dateto the header set beforecomputeSigV4Signature. (b) Presigned (query-signed):computeSigV4Signatureunconditionally injectedx-amz-dateinto the canonical header block — correct for CRUD but wrong for presigned, wherex-amz-dateis theX-Amz-Datequery param. The request declaredX-Amz-SignedHeaders=hostwhile the signature coveredhost;x-amz-date→ every worker-minted presigned URL 403s. Found when the worker first needed to mint athumbnailUrl(finalize now mints a 15-min signed GET of the 480 artifact, which the suite asserts returns 200). Fixed with an opt-inSigningInput.signAmzDateHeader(default true = CRUD unchanged, thes3-signunit test stays green; the presign path passesfalse). Proven against real MinIO: presigned GET 200 + CRUD CRUD. - The temp dir must be world-traversable (0755) for ClamAV. The clamd
daemon runs as user
clamav, the worker as another user; a0700temp dir (themkdirdefault) is not traversable by clamd, sozCONTSCANaccepts the connection and then times out (every scan fails with "clamav scan timeout").createTempFilenowchmod 0755the temp dir. Files are random-UUID named and reaped within seconds, so the transient cross-user readability is the documented trade-off. (This is the single most likely cause of a "clamav scan timeout" in any new deployment.) sharplimitInputPixelsis a constructor option, not a pipeline method —sharp(path, {limitInputPixels: N}), not.limitInputPixels(N)(the latter is aTypeError). The ESM type surface also lags the runtime, so sharp is consumed via a structuralSharpInstanceinterface (lib/sharp.ts).zVERSIONhangs on both unix socket and TCP (clamd 1.5.4); Dockerclamav/clamavimages hang the TCPINICcommands (PING works) while the unix socket works.getClamdVersionis best-effort (3 s timeout →"unknown"); the CI Docker profile should prefer the unix-socket mount.- Pre-existing worker red NOT introduced here (recorded, not fixed):
worker tsc43 (includeslib/s3.ts2 —exactOptionalPropertyTypesdrift — and ones3-sign.test.tsheader-casing unit that fails onContent-Typevscontent-type, verified pre-existing at the Phase-9 start commit via a stashed diff);lib/s3.tsalso carries one pre-existing eslintprefer-const(errorFrom'slet message). All of TASK-0102's NEW files are 0 tsc errors / 0 eslint errors.

TASK-0103 (File API + upload UX + rendering + picker + browser) recorded decisions & conflicts:
-
Spec's assumed web paths don't exist — adapted to the actual structure (rule 29, recorded not guessed). The spec names
apps/web/src/app/(workspace)/files/page.tsx,components/blocks/FileBlock.tsx,components/files/UploadProgress.tsx. The repo has no(workspace)route group (routes live under(app)/w/[workspace]), nocomponents/blocks/dir (blocks render fromcomponents/editor/block-renderer.tsx), and the upload-progress UI is inlined in the file block. So: the route isapp/(app)/w/[workspace]/files/page.tsx;FileBlocklives atcomponents/files/FileBlock.tsxand is wired throughblock-renderer.tsx'scase "file"(the old placeholder renderer was deleted — it read non-existentfileName/fileSize/fileTypeprops); progress isUploadProgressCardinsideFileBlock.tsx. Functionality is identical to the spec; only file placement differs. -
docs/api/files.md§1–7 is a pre-Phase-9 DRAFT describing a different contract (POST /files,POST /files/:fileId/complete, rawkeyin the file object,GET /files?mimeType=,GET /files/:id/download). The implemented API differs (endpointsPOST /files/uploads+PUT /files/uploads/:id/complete; the file object never exposes the rawkey; list paramsscope/kind/status/search/sort/cursor). TASK-0101's DoD marked the doc "merged" without reconciling it. Rather than rewrite the whole draft, an authoritative TASK-0103 addendum (§8) was added at the bottom (written againstapps/api/src/modules/files/*+ the integration suite) and a banner at the top marks §1–7 as draft. This is the conflict between the doc and the implementation, recorded here. -
BlockProperties.widthPercentadded (a Phase-4 block-schema extension per spec §Backend). The existingwidthPctfield belongs to the image/video block types (10–100 %); the spec names the file-block resize fieldwidthPercent(25–100 %).packages/editor's codec passespropertiesthrough with no strict zod, so no codec change is needed — the field is validated only in the UI (clamped). No DB schema change (block JSON carries it). -
Legacy
fileApiremoved;uploadFileis now an adapter. The oldlib/api.tsfileApiobject (POST /files/upload,POST /files/:id/complete,GET /files/:id/url,GET /workspaces/:id/files) 404/405'd against the live API and had zero external consumers (only the olduploadFileused it). It was removed (never silently — it was already dead). TheuploadFileexport is preserved as a thin adapter overlib/files/upload.tsreturning the legacyFileMetashape, which fixes the three pre-existing broken consumers that imported it (insert-dialog image tab, form FILE field, database cell file upload) — they now upload through the real presigned flow. This is backward-compatible: the adapter falls back toclaims.workspaces[0]when no workspace header is passed (the old routing), and passes the 500 MB hard ceiling as the pre-check limit (the legacy contract had no client pre-check; the server still enforces the plan limit authoritatively). -
List permission model is workspace-level
VIEWfor now; per-page private-page filtering is TASK-0104.GET /filesgates on the workspaceVIEW(non-member → 403; member → whole-workspace list). Server-side per-pageVIEWfiltering (files in a private page absent for non-viewers) is explicitly TASK-0104's scope per the spec; the UI renders only what the server returns, and a stale-URL file renders the "No access" card from the block renderer. Recorded so the current scope is not read as the final permission model. -
Web baselines (recorded, not fixed).
apps/webtsc --noEmithad a 210-error pre-existing baseline (large, from Phase 2–8); this task's new/modified files are 0-error and the total dropped to 207 (removing the dead placeholder FileBlock removed 3).jsdomis not installed, so the pure-logic unit tests use the// @vitest-environment nodebanner.@factory/webeslint is red at baseline (scopednext lintcan't load thereact-hooks/@next/nextplugins → "Definition for rule … not found" noise); this task's files have 0 real eslint issues (verified by filtering that noise). -
Worker S3 bucket client built from the raw legacy
S3_BUCKET(fixed). The TASK-0102 refactor moved the files bucket toS3_BUCKET_FILES(factory-files) withS3_BUCKETkept as a legacy alias, and the API'smodules/files/s3.tsreadsenv.S3_BUCKET_FILES || env.S3_BUCKET. Butapps/worker/src/index.tsstill constructed its mainS3Clientfrom the rawenv.S3_BUCKET. A dev.envcarrying the stale pre-Phase-9S3_BUCKET=factory-uploads(that bucket no longer exists) therefore sent the pipeline'sdownloadTo/putObjectto a missing bucket → every upload faileddownloadTo(...) HTTP 404while the API presigned intofactory-files(the object landed in the right place; only the worker looked in the wrong bucket). Fixed by mirroring the API:bucket: env.S3_BUCKET_FILES || env.S3_BUCKET. Recorded as a requirement conflict between the TASK-0102 config split and the worker's client construction (rule 29). Verified end-to-end after the fix: real PNG → PUT 200 → completeSCANNING→ pollREADY. -
E2E (
tests/e2e/specs/files.spec.ts) — the existing spec +utils/api.ts uploadFilewere stale (pre-Phase-9 contract) and app-wide selector testids are unimplemented, so this task rewrote them to the real contract and shipped the files testids. The old spec targeted dead routes (POST /files/upload-sessions,POST /files/:id/complete), a dead URL shape (/w/<ws>/p/<id>; real route is/w/<ws>/page/<id>), read the page id from a{page}wrapper that doesn't exist (create returns the bare page), and asserted testids (file-card-*,file-input,toast-error) thatapps/webnever shipped. The repo's own selector table intests/README.mdis unimplemented app-wide (only 17 ad-hoc testids exist; 6 of 16 specs reference the stale ones — a pre-existing, not TASK-0103, gap). This task: (a) added the canonical files testids to the web UI (file-inputon the picker block input,file-card-{id}/file-name-{id}/file-download-{id}onFileBlock,editor-rooton the editor canvas which is also the drop target); (b) rewroteuploadFile()to the real presigned flow (POST /files/uploadswith thex-factory-workspaceheader → PUT the bytes with the exact signedContent-Type→PUT /files/uploads/:id/complete→GET /files/:idfor status + signeddownloadUrl/previewUrl); (c) rewrotefiles.spec.tsto the real routes/testids/response shapes. The full security suite (500 MB cap, EICAR/malware, MIME spoof, path traversal) is TASK-0105's scope per the spec; this spec covers the upload→render→download happy paths + the oversized + unauthorized cases. -
⚠️ Block-type case mismatch between the REST wire and the editor codec (defect found by the render E2E; fixed at the REST boundary, rule 29). The DB/REST block type is the UPPERCASE
BlockTypeenum (FILE,PARAGRAPH, …); the web editor's codec is lowercase (file,heading_1, …). The realtime gateway already reconciles the two at its boundary (apps/realtime/src/prisma-store.tscodecToDbTypeon write /dbToCodecTypeon read), but the REST path did not: a block created viaPOST /pages/:id/blockswith the codec case ("file") failed the zodz.enum(BLOCK_TYPES)→ 400, and a REST-createdFILEblock reachedblock-renderer.tsxstill uppercase, socase "file"never matched and the file block rendered nothing (the 45 sfile-card-{id}E2E timeout). Fix (mirrors the existing gateway pattern, backward compatible): (a) API write-normalize —apps/api/src/modules/blocks/dto.tsblockTypeSchematransforms the input totoUpperCase()before thez.enum(BLOCK_TYPES)pipe, so bothfileandFILEare accepted and the DB case is stored; UPPERCASE input is byte-for-byte unchanged (existing integration tests send/expectPARAGRAPH/FILE). (b) Web read-normalize —apps/web/src/lib/api.tsmapBlocks(used bypageApi.blocks+blockApi.list) maps the DB case to the codec case for known values (unknown values pass through), so the renderer'scase "file"matches. Mutations are intentionally untouched: the client already sends the codec case, and (a) accepts it. Evidence: livetype:"file"→ 201 storedFILE/type:"FILE"→ 201 /type:"not_a_type"→ 400; the render E2E (image + PDF + video + audio + doc + virus) is green; files integration 20/20 unchanged; API tsc 85 and web tsc 210 pre-existing baselines unchanged (0 new errors). -
⚠️ Pre-existing editor-insert gaps block the UI-driven upload E2E (recorded, not fixed — out of TASK-0103 scope, the editor's insert pipeline is Phase 4's).
blockApi.insertinapps/web/src/lib/api.tstargetsPOST /blocks, which does not exist (verified live: 404Cannot POST /api/v1/blocks; the route never existed —git log -Son the blocks module shows onlypages/:pageId/blocks). The working route isPOST /pages/:pageId/blocks. This dead route is reached by the editor'suseInsertBlock(slash menu), the context menu, the sidebarFilePickerinsert, AND the drag-drop + paste upload paths (page-view.tsx→useFileUploads.startwithoutupdateBlockId). Separately, the tiptap typing/Enter sync path targetsPOST /blocks/batch, which returns 400VALIDATION_ERROR(verified live in-browser, 2026-09-29: focus a rendered paragraph, type, press Enter → 0 new blocks; the only block POST the browser emits isPOST /blocks/batch → 400). So as of this run the web editor cannot create any block through the browser. Companion latent defects on the client insert path (each independently 4xx's today): (a) a fresh page seeds its default paragraph at position"A"(apps/api/src/modules/pages/page.service.ts), while the client's LexoRank grammar (packages/editorlowercase base-36) rejects"A"as invalid — the API'spositions.tsuses full base-62, so the server side is fine, only the client's round-trip breaks; (b)inline-editor.tsxsaves blockcontentas an HTML string, but the API validatescontentasz.record→ 400; (c) client-localblk_…ids collide with the API's seededblk_…ids. Consequence for THIS task's E2E: the drag-drop and paste block-insert steps cannot be E2E-verified until the editor insert pipeline is repaired, sofiles.spec.tscreates its rendered file blocks through the workingPOST /pages/:id/blocksroute (REST), and E2E-verifies the upload path (c) "Add file" button end-to-end in the browser — it routes throughblockApi.update(PATCH /blocks/:id, which exists) withupdateBlockId, so no insert is needed. The shared upload service (lib/files/upload.ts) — the part common to all three paths — is unit-tested (23/23) and the button-path browser round trip (session → XHR PUT to MinIO → complete → block re-render) is E2E-proven. ThePOST /blocksroute +POST /blocks/batch400 + position/content/id reconciliation belong to the editor task (Phase 4 ownership); repairing all of it here would be an unscoped, unverified change with whole-editor blast radius (rule 29: record the conflict; rule 30: do not mark complete on unverified work). Scope decision (2026-09-29): recorded as a Phase-4 follow-up and Phase 9 proceeds; when the editor insert pipeline is repaired it will unblock E2E verification of upload paths (a) and (b) and the editor's own slash/Enter/context-menu block creation. -
⚠️ Upload-session
blockId: null→ 400 (Phase-9 contract bug, fixed). The web upload client (apps/web/src/lib/files/upload.ts) always sendspageIdandblockIdin thePOST /files/uploadsbody, defaulting either tonull(blockId: options.blockId ?? null). The API'sCreateUploadDtodeclared themz.string().min(1).optional(), which rejects an explicitnull(.optional()only allows the key to be absent). So every browser upload — the "Add file" button (path c), drag-drop (a), and paste (b) — 400'd at session creation withVALIDATION_ERROR: blockId Expected string, received null, and the block never got itsfileId. The API'suploads.service.tsalready treatsnullas "none" (if (blockId)/pageId ?? null), so the DTO was the sole defect. Fixed by makingpageId/blockId/replaceFileIdz.string().min(1).nullable().optional()(a string or a real id input is unchanged — backward compatible; the E2E/API helper sends them omitted). Proven live: browserPOST /files/uploads{blockId:null}→ 201 session → browser XHRPUTto MinIO 200 →PATCH /blocks/:id{type:"file", properties:{fileId,name}}→ 200 →complete→READY, block re-renders as the image (the path-c E2E). -
Video E2E uses webm/VP9, not mp4 (test-env codec, recorded). The spec's video acceptance names
mp4; Playwright's Chromium is an open-source build that ships without the H.264 decoder, so an mp4's metadata never loads there (canPlayType("video/mp4; codecs=\"avc1…\"")==="",<video>errorsMEDIA_ELEMENT_ERROR: Format error, videoWidthstays 0). The product renders a native<video>for any video mime, andvideo/webmis itself a spec-listed kind (mp4/webm/ mov). The E2E therefore uploads a small VP9.webmfixture (verified: ChromiumcanPlayType("video/webm; codecs=\"vp9\"")==="probably",loadedmetadatafires,videoWidth === 640). This is a test environment limitation, not a product defect — an H.264 mp4 renders and plays in a production browser; the E2E asserts the render + decode with a codec the test browser supports. -
E2E auth rate-limit (test-infra, not a product change). The suite performs ~11 fresh auth calls from one loopback IP in <60 s (one demo login per test in the
apifixture + one in theauthPagebrowser context + thesecondUserregister/login). The product defaultRATE_LIMIT_AUTHis 10/min per IP (config/env.ts), so the final test's register/login 429'd (RATE_LIMITED) once the render tests stopped timing out at 45 s (the fast green tests bunch the auth calls into one 60 s window). A throwaway test IP is not a brute-forcer, so the e2e environment overridesRATE_LIMIT_AUTH=100000(local restart script- the CI e2e job's
.envstep), mirroring the existingRATE_LIMIT_API=100000CI pattern. Production keeps the 10/min default.

⚠️ Quarantined-file detail endpoint returned 403 (Phase-9 defect, fixed). The spec's virus path ("red 'Blocked by virus scan' card, no download affordance") requires the client to receive
GET /files/:idwithstatus: VIRUS_DETECTED— but the sharedrequireReadablethrewFILE_QUARANTINED(403) for every caller, detail endpoint included, so the card was unreachable (the block fell through to the "no access" ErrorCard instead). This contradicted the documented contract:docs/api/files.md§8.5 says the detail endpoint returns the 200 record forVIRUS_DETECTED(withscanReport, no signed URLs — "downloads return 403"), and only §8.6 (/download//preview) 403. Fix (code now matches the doc):requireReadablegained anallowQuarantinedflag (detail path only);get()returns the record withoutdownloadUrl/previewUrl(fields omitted, never signed — the bytes are still never served:/downloadand/previewkeep the 403). WebFileDetailtypes those fields as optional; therefetchIntervalre-mint and the twolocation.hrefaction sites guard for absence (unreachable for a quarantined file — the red card renders before any URL branch). Verified live: EICAR → detail200 {status:"VIRUS_DETECTED"}with no URL fields;/download//preview→403 FILE_QUARANTINED; browser E2E renders the redrole="alert"card. (Integration-suite gap recorded: no integration test asserted the detail-endpoint status for a quarantined file, which is how the defect survived — see TASK-0103 "Unit + integration + E2E green" below.)

Drag-drop (a) + paste (b) share the same unit-tested (23/23) upload service and the same pre-check/progress/ cancel UX; their final block-insert step is blocked by the recorded pre-existing editor block-creation defect (Phase-4 follow-up, notes above), so the upload-to-READY round trip is verified but the insert is not yet E2E-verifiable until that pipeline is repaired. TASK-0101
Title
Storage integration: presigned upload/download sessions, MIME magic-byte validation, workspace key layout, signed GET URLs
Objective
Wire packages/storage into the API files module and the worker so that
uploads happen as plan-limited presigned PUT sessions with magic-byte MIME
verification, objects land in a private per-workspace key layout, and
downloads are served via short-lived (15 min) signed GET URLs behind a
server-side permission check.
Background
packages/storage scaffolds an S3 client (src/client.ts), a key layout
helper (src/keys.ts), MIME magic-byte validation (src/mime.ts), and a
thumbnail interface (src/thumbnails.ts). MinIO is provisioned by Phase 0
(Docker) and is S3-compatible. The Prisma File model exists
(prisma/schema.prisma: files — workspaceId, blockId?, pageId?,
name, key @unique, url?, size Int, mimeType, status FileStatus @default(UPLOADING), metadata Json, thumbnailKey/Url, scanReport Json?,
uploadedBy, permissions Json?, deletedAt; plus FileVersion
file_versions). Size limits: free plan 50 MB per file, paid 500 MB per
file — sourced from Workspace.usageLimits (Json; default shape
{ maxFileSize: 52428800, storageQuota: 1073741824 } for free,
{ maxFileSize: 524288000, storageQuota: 10737418240 } for paid; the
effective limit = min(usageLimits.maxFileSize, plan default)).
LIMITS.MAX_FILE_SIZE = 500 MB is the hard ceiling
(packages/shared/src/constants.ts). The files module scaffold is
apps/api/src/modules/files/.
Requirements
- Storage config:
@factory/configenv —S3_ENDPOINT,S3_REGION,S3_ACCESS_KEY_ID,S3_SECRET_ACCESS_KEY,S3_BUCKET_FILES(private bucket),S3_BUCKET_THUMBS(read-only serving bucket, still server-authorized),S3_SIGN_EXPIRY_SECONDS=900,CLAMD_SOCKET(optional). Bucket policy: no public access; both bucketsPrivateACL. - Key layout (
packages/storage/src/keys.ts):files/{workspaceId}/{fileId}/{version}/{safeName}andthumbs/{workspaceId}/{fileId}/{size}/{safeName}.safeName= original basename with all path components stripped (path.basename, then replace[\x00-\x1f\x7f<>:"\\/*?|]with_, max 180 chars, extension preserved case-insensitively); collisions get a-<8 hex>suffix. Bucket naming per deployment: one bucket per environment, all workspaces shared inside it (workspace isolation is by key prefix + permission checks, NOT by bucket ACL — document this decision). - Upload sessions:
POST /api/v1/files/uploads{ name, size, mimeType, pageId?, blockId?, replaceFileId? }→ 201{ upload: { file: FileRecord (status UPLOADING), uploadUrl, method: "PUT", headers: { "Content-Type": <validated mime>, "Content-Length": size }, expiresAt } }. Server: authenticates + checksEDITon the target page/block (orEDITon workspace root for unattached), enforcessize ≤ effective maxFileSize(413FILE_TOO_LARGEwith the limit in the message body) andsize ≤ remaining storageQuota(413QUOTA_EXCEEDED), creates theFilerow, and signs a PUT URL (15 min, content-length-locked when the S3 SDK supports it). - MIME validation: the client-claimed
mimeTypeis checked against the allow-list (images: png/jpg/webp/gif/svg; documents: pdf/docx/xlsx/pptx/ md/txt/csv; archives: zip; video: mp4/webm/mov; audio: mp3/wav/m4a; other: application/octet-stream) viapackages/storage/src/mime.ts; SVG uploads are restricted totext/plain-treated (served withContent-Disposition: attachment+X-Content-Type-Options: nosniff— see Security). OnPUT /api/v1/files/uploads/{uploadId}/completethe server HEADs the object, then downloads the first 8 KiB and runssniffMagicBytes(buffer)frompackages/storage/src/mime.ts: mismatch with the claimed type → status FAILED, errorMIME_MISMATCH, object deleted, session closed (integration test in TASK-0105). - Signed GET:
GET /api/v1/files/{fileId}(metadata, includesdownloadUrl+previewUrlwhen available, 15-min signed URLs) andGET /api/v1/files/{fileId}/download(302 → signed URL) both require the caller to have ≥ VIEW on the file (TASK-0104 defines file permission resolution; here: ≥ VIEW on the containing page/block, or explicit file grant). URLs are signed for exactly one object key, expiry 900 s, and includeresponse-content-disposition(attachment for download, inline for preview of image/video/audio/pdf). - Upload completion:
PUT .../completeverifies object existence + size matches the session (422UPLOAD_INCOMPLETEotherwise), setsFile.status = SCANNING, enqueuesfilesqueue jobprocess-file{ fileId, version: 1 }, and closes the session.POST .../canceldeletes the partial object + row (status FAILED,deletedAtset). - Concurrency: one active upload session per (file, version); duplicate
sessions for the same
replaceFileIdare allowed but each is an independent version (see TASK-0104).
Acceptance Criteria
- Integration: creating an upload session with
sizeabove the free-plan limit returns 413FILE_TOO_LARGEnaming 50 MB; the same size for a paid workspace returns 201 (plan boundary proven). - Integration: presigned PUT with the wrong
Content-Typeor a size ≠ declared size fails completion withUPLOAD_INCOMPLETE/422 and no READY file row. - Integration: a 1 KB PNG claimed as
image/pngcompletes to SCANNING; a text file claimed asimage/pngfails withMIME_MISMATCHand the object is deleted from the bucket (assert via storage client list). safeNameunit matrix:../../etc/passwd→passwd;a\0b.png→a_b.png-style sanitized; 250-char name → ≤ 180 chars with extension kept;UPPER.PNGkeeps its extension.- Signed GET:
GET /api/v1/files/{id}/downloadby a non-member (no VIEW) → 403FORBIDDEN+ audit eventfile.download_denied(audit event added in TASK-0104; asserted here as part of the permission check stub); by a member → 302 to a URL that (a) works once, (b) 403s from S3 after 900 s, (c) 403s against a different object's key (key binding). - Upload session URL expires at 900 s: PUT after expiry fails (S3
signature expired) and completion reports
UPLOAD_EXPIRED. - Two buckets verified private: anonymous S3
GETon the raw object key (unsigned) returns 403 for bothfilesandthumbskeys.

Database Changes
- No schema change (File/FileVersion exist). Migration: ensure
files.statusdefaultUPLOADING+files.keyunique (present). workspace.usageLimitsdefaulting: backfill migration that sets{ maxFileSize, storageQuota }perbillingPlanfor workspaces where the JSON is{}(free defaults) — data migration, reversible (no-op on re-run).
API Changes

Tests
- Unit:
sanitizeFilenamematrix, sniffMagicBytesfor 12 signatures, key layout builders, effective-limit resolver (plan × usageLimits). - Integration: all Acceptance Criteria scenarios (sessions, MIME mismatch,
expiry, signed URL binding, bucket privacy) in
tests/integration/files-storage.spec.tsagainst the Docker MinIO.
Security
- No public bucket access; every object is reachable only via signed URL or server redirect; raw key is never returned to clients.
- Magic-byte sniffing is authoritative over the client claim; mismatch = failure, never "trust and warn".
- SVG served as attachment only (no inline render) — stored XSS vector closed at the storage layer, not just the UI.
- Path traversal: keys are server-built from
fileId; the clientnamenever reaches the key path unsanitized (unit matrix proves it). - Secrets from env only; presigning uses the server-side S3 credentials, which the browser never sees.

TASK-0102
Title
File pipeline worker: status machine, ClamAV scan, metadata extraction, thumbnails, file.uploaded event
Objective
Complete the files queue processor so every uploaded object transitions
UPLOADING → SCANNING → READY | VIRUS_DETECTED | FAILED with a ClamAV scan,
extracted metadata (mime, size, sha256, image dimensions / video duration /
PDF pages), and generated thumbnails (images in 3 sizes, PDF first page,
video first frame) — emitting file.uploaded and updating the search index
on success.
Background
The upload session (TASK-0101) leaves the File row in SCANNING and
enqueues process-file on the BullMQ files queue
(apps/worker/src/queues.ts). Prisma FileStatus enum: UPLOADING, SCANNING,
READY, VIRUS_DETECTED, FAILED; File.metadata (Json) holds dimensions /
duration / pages / hashes; File.scanReport holds the ClamAV result;
FileVersion records each stored generation. packages/storage/src/
provides thumbnails.ts (interface) and the S3 client. sharp (images),
pdfjs-dist or mupdf (PDF rasterization), fluent-ffmpeg (video first
frame) are the candidate libraries — each must be justified per AGENTS.md
rule 17 and pinned in apps/worker/package.json. Domain events:
file.uploaded in @factory/shared/events. The search indexer
(TASK-0096) consumes file.uploaded to index file docs.
Requirements
- Processor:
apps/worker/src/processors/files/processor.tswith jobprocess-file{ fileId, version }; at-least-once safe (re-entrant: re-running a READY file's job is a no-op; re-running after FAILED is an explicit retry). Steps run sequentially, each wrapped with timing + structured logs (file:{fileId}:step:{name}), and each step's failure classifies as retryable (network, S3 5xx) or fatal (corrupt object, unsupported format) → retryable: BullMQ backoff; fatal: status FAILED +errorrecorded inmetadata.pipeline.error. - Status machine (only valid transitions, enforced in the processor): UPLOADING → SCANNING (set at complete, TASK-0101) → READY | VIRUS_DETECTED | FAILED. READY → FAILED only on later re-scan. Any other transition throws and is logged as a pipeline invariant violation.
- Virus scan: if
CLAMD_SOCKETis configured, runclamdscanover the local temp copy (download to temp, scan, delete temp — object never stays on disk > scan duration + 5 min cleanup cron); INFECTED → status VIRUS_DETECTED,scanReport = { engine: "clamav", version, infected: true, signature }, NO thumbnail generation, NO search index update, file.uploadedNOT emitted; emit domain eventfile.virus_detected(add to@factory/shared/events) and notification to the workspace admin (via Phase-7 notification service, stub call if module absent). IfCLAMD_SOCKETis unset: dev mode — logWARN file scan skipped (CLAMD_SOCKET not configured)and continue, scanReport = { engine: "none", skipped: true, reason }; inNODE_ENV=productiona missing socket is a boot error (worker refuses to start — fail closed in prod, documented in the runbook). - Metadata extraction (streaming where possible): mime (re-sniff from
bytes, final authority), size (S3 head), sha256 (streaming hash,
metadata.sha256), images: width/height/orientation viasharp(metadata.dimensions); video: duration seconds + codec viaffprobe(metadata.media); PDF: page count viapdfjs-dist(metadata.pdf = { pages }). Unsupported/undecodable → keep mime as declared,metadata.pipeline.warnings[], still READY (a file is downloadable even without rich metadata — only thumbnails are affected). - Thumbnails (write to
thumbs/key layout viapackages/storage/src/thumbnails.ts): images → 3 sizes (160, 480, 1280 px wide, same format, max 1280) at keysthumbs/{ws}/{fileId}/{160|480|1280}.{ext}; PDF → first page PNG (1280w)- page count; video → first frame PNG (1280w); audio/other → no
thumbnail (
thumbnailKeynull).File.thumbnailKey= the 480 variant (primary UI thumb);File.thumbnailUrl= signed 15-min GET (regenerated on read, never stored long-lived).
- page count; video → first frame PNG (1280w); audio/other → no
thumbnail (
- On success: status READY,
metadata.pipeline = { scannedAt, sha256, durationMs, steps: [...] }, emitfile.uploadeddomain event{ workspaceId, fileId, name, mimeType, size, blockId?, pageId? }→ search indexer indexes the file doc (TASK-0096); recordFileVersion { version, key, size, createdBy }(created by TASK-0101 on complete; here only verify + backfill on re-runs). - Job tuning: concurrency 4 (env
FILES_WORKER_CONCURRENCY),removeOnComplete: 500,removeOnFail: 200, attempt limit 3 with exponential backoff (2 s base), temp-file cleanup cron every 5 min (delete files older than 10 min in the temp dir), disk guard: abort with fatal error if temp dir free space < 512 MB.
Acceptance Criteria
- Integration: upload a 2 MB PNG → within 10 s status READY,
metadata.dimensionscorrect,metadata.sha256matches the local hash, 3 thumb keys exist in the bucket, thumbnailUrlreturns 200 with the right image signature, file.uploadedobserved on the Redis stream. - Integration: EICAR string file → status VIRUS_DETECTED (ClamAV enabled in
the CI Docker profile),
scanReport.infected = true, zero thumb keys created, nofile.uploadedon the stream, admin notification enqueued (assert notification row/job). - Dev mode: with
CLAMD_SOCKETunset (default local env) the same EICAR file reaches READY withscanReport.skipped = trueand the WARN log present (log assertion) — and boot inNODE_ENV=productionwithout the socket throws (unit test on the worker bootstrap). - A 50 MB MP4 →
metadata.media.durationwithin ±0.5 s of ffprobe truth, first-frame PNG exists. A 20-page PDF →metadata.pdf.pages = 20, first-page PNG exists. - Corrupt object (zip bytes claimed as zip, truncated): fatal classification
→ status FAILED,
metadata.pipeline.errorpresent, no retries beyond attempt 3 (assert attempts = 3 in the queue job info). - Re-runnability: kill the worker mid-pipeline (SIGKILL between scan and
thumbs), restart, the job resumes to READY without duplicate thumbs
(key-idempotent) or duplicate
file.uploaded(event dedupe: only emit when status actually transitions to READY). - 50 concurrent
process-filejobs: temp dir peak usage bounded (assert disk guard config respected), no job starvation > 60 s in the local test.
Backend Changes
apps/worker/src/processors/files/processor.ts+ steps:steps/scan.ts,steps/metadata.ts,steps/thumbnails.ts,steps/finalize.ts;files/temp.ts(temp dir mgmt + cleanup cron); job types inapps/worker/src/queues.ts.packages/storage/src/thumbnails.ts: implementgenerateThumbs(image/pdf/video adapters) +ThumbsResult.apps/worker/package.json: addsharp,pdfjs-dist(ormupdf— decision recorded),fluent-ffmpeg+ systemffmpeg/ffprobein the worker Docker image (infrastructure/docker/Dockerfile.worker),clamavin the CI Docker profile (infrastructure/docker/docker-compose.yml).@factory/shared/events: addfile.virus_detected.@factory/config:FILES_WORKER_CONCURRENCY,FILES_TEMP_DIR,CLAMD_SOCKET(validated).

None (worker only; rendering is TASK-0103).
Database Changes
- No schema change. Data: none.
API Changes
- None (queue-driven). The
FileJSON returned byGET /api/v1/files/:idgainsmetadata.pipelinefields (documented indocs/api/files.md).
Tests
- Unit: status machine transition table (all invalid pairs throw), error classification (retryable vs fatal matrix), temp cleanup logic, sha256 stream helper.
- Integration (
tests/integration/files-pipeline.spec.ts): all Acceptance Criteria scenarios incl. EICAR (CI Docker profile with ClamAV), corrupt file, re-run after kill, concurrent burst. - Fixture files:
tests/fixtures/files/(small png, 2-page pdf, 3s mp4, eicar.txt — EICAR is a standard test string, not malware).
Security
- ClamAV mandatory in production (boot failure otherwise) — fail closed.
- Scanned objects handled in a temp dir on a filesystem mounted noexec; temp files deleted regardless of outcome (finally block + cron backstop).
- Virus-detected files remain stored (forensics + audit) but are never
signed for download (TASK-0103/0104 enforce: download → 403
FILE_QUARANTINED), never thumbnailized, never indexed. - Metadata parsers run on untrusted input: timeouts (10 s per step), memory
cap on decoded images (sharp
limitInputPixels), PDF page cap 5 000 (beyond → warning, no rasterize), video probe cap 30 s.
Documentation
docs/development/files-pipeline.md(new): pipeline diagram, step semantics, ClamAV setup (Docker + local socket), retry/backoff policy, temp dir + disk guard, how to add a new thumbnail kind.docs/architecture/storage.mdaddendum: pipeline state machine + event contract (file.uploadedpayload,file.virus_detectedpayload).
Dependencies

TASK-0103
Title
File API + upload UX (drag-drop, paste, block) + file block rendering + file picker + file browser
Objective
Let users put files into pages: drag-drop from the OS, paste an image, or insert via the file block button — with upload progress — and render every file kind correctly (image inline + resizable + caption, native video/audio players, PDF preview, document download card), plus a file picker in the page sidebar and a file browser.
Background
Upload sessions (TASK-0101) and the pipeline (TASK-0102) exist: the client
gets a presigned uploadUrl, PUTs the blob, calls complete, then polls /
subscribes for status. The editor (Phase 4) has block types incl. file
(BlockType enum in Prisma) and the slash menu; packages/editor holds the
block model. Signed URLs (15 min) come from
GET /api/v1/files/:id and .../preview. The file picker and browser are
plain Next.js pages/components under apps/web/. File.metadata carries
dimensions, media, pdf, sha256, pipeline.
Requirements
- Upload paths (all → one
uploadFile(file, { pageId, blockId?, replaceFileId? })service inapps/web/src/lib/files/upload.ts): (a) drag-drop from OS onto the page canvas → drop indicator, inserts afileblock at the drop position; (b) paste (Ctrl+V) an image from the clipboard → image block at the cursor; (c) file block "Add file" button → native file input (multi). Each path: size pre-check against the plan limit (fetched once per page session) with a clear error toast ("50 MB per file on your plan"), then presigned session → XHRPUT(notfetch: progress events) with a progress bar on the block (0–100 %, speed, cancel button) →complete→ status polling (2 s, max 60 s) → block switches to rendering. - Upload failure UX: presigned PUT 403 (expired) → auto-retry once with a
fresh session; MIME mismatch / quota / virus → error state on the block
with the server message + "Remove" and (for recoverable quota) "Upgrade"
hint; cancel mid-upload →
cancelAPI + block removed (or "Upload failed — retry" if the user prefers; default: remove). - File block rendering (
apps/web/src/components/blocks/FileBlock.tsx): image: inline, max-width 100 %, resizable via bottom-right drag handle (50/75/100 % presets + free drag, stored on the block aswidthPercent), caption toggle below (inline editable text); video: native<video controls>(poster = thumbnail); audio: native<audio controls>+ icon; PDF: preview mode — iframe with the signed preview URL (15-min refresh on expiry: re-fetch before use whennow + 60 s > expiresAt) + "Open in new tab" + "Download"; document/other: download card — file-type icon (by mime), name, size (human), version count (if > 1), Download button (302 endpoint), "Copy link" (share link =GET /files/:id— public only if the page is shared; otherwise "link requires access" tooltip). While status ≠ READY: skeleton with status label (Scanning…, Processing thumbnails…); VIRUS_DETECTED: red card "Blocked by virus scan — contact an admin" (no download affordance). - File picker (page sidebar): "Files" tab listing files attached to this page tree (workspace-scoped filter chip: "In this page / In workspace"), grid (thumbnail, name, size, uploader, date), click → insert as file block at cursor; search box (client filter by name); empty state ("No files here yet — drag one in").
- File browser: route
apps/web/src/app/(workspace)/files/page.tsx— table of workspace files (name, kind icon, size, uploader, page, status, updated), sort (name/size/date/updated), filter by kind (image/video/audio/pdf/doc/other) + status, row click → preview modal (image lightbox / pdf iframe / video player / download card) + "Open page" link; pagination 50/page (cursor); empty + error states; mobile: card list instead of table. - Permissions in UI: items the user lacks VIEW on are absent (server filters); a file in a private page the user can't open renders "No access" card if reached via stale URL.
- Accessibility: upload progress announced via
aria-live="polite"(25 % steps), cancel is a real button, video/audio players have text alternatives (name + duration), picker grid keyboard-navigable (roving tabindex), download card is a<button>not a styled div.
Acceptance Criteria
- E2E (
tests/e2e/files.spec.ts): drag a PNG from the OS (PlaywrightdragAndDropwith a fixture file) onto the page → block appears → progress bar visible → renders as inline image with caption toggle working. - E2E: paste a screenshot (clipboard API injection) into the editor → image block at cursor renders.
- E2E: upload a PDF → block shows PDF preview (iframe) + Download works
(download event with the right filename); video mp4 →
<video>plays (assertvideoWidth > 0afterplay()). - E2E: oversized file on free plan (60 MB fixture) → pre-check toast with the 50 MB limit, no request sent to the API (assert zero upload session calls).
- Progress: a 20 MB upload shows monotonically increasing progress and completes (XHR path proven by intercepting the PUT and confirming chunked responses); cancel at ~50 % → session cancelled (file row not READY, block removed).
- File picker: after 2 uploads, the sidebar picker shows both with thumbnails; clicking inserts a block referencing the file id.
- File browser: 3 files across 2 pages → table lists all 3 with correct page links; kind filter "image" shows only the PNG; row click opens the preview modal (image lightbox at full size).
- Virus file (EICAR uploaded with ClamAV CI profile) → block shows the red blocked card; no download button in the DOM.
- Mobile (390 px): upload via block button works; browser is a card list; no horizontal overflow.
Backend Changes
GET /api/v1/files?scope=page|workspace&pageId=&kind=&status=&cursor=list endpoint (server-side permission-filtered, 50/page) inapps/api/src/modules/files/files.controller.ts+ service query.GET /api/v1/files/:idextended: returnsexpiresAtof the signed URLs +versions[](id, version, size, date) +captionsupport (caption lives on the block, not the file — no file change).- Block model:

Database Changes
- No schema change (block JSON carries width/caption).
API Changes
- New list endpoint
GET /api/v1/files(params per Requirements §5);GET /api/v1/files/:idresponse extended. Documented indocs/api/files.md.
Tests
- Unit: upload state machine (session retry, cancel, progress monotonicity, poll timeout), block renderer kind resolution (mime → kind matrix), picker filter/sort logic.
- Integration: list endpoint permission filtering (private page files absent for non-members), kind/status filters, cursor.
- E2E: all 9 Acceptance Criteria scenarios.
Security
- Signed URLs are the only fetch path for content; the UI never assembles bucket URLs from raw keys.
- PDF preview iframe:
sandbox="allow-scripts allow-popups"+referrerpolicy="no-referrer"; signed URL expires 15 min. - No client-side MIME trust for rendering decisions that affect safety:
the kind is derived from the server-verified
mimeType(post-scan), not the extension. - Download filename comes from the server record (sanitized at upload), never client-constructed.
Documentation

Dependencies
- TASK-0101 (sessions/signed URLs), TASK-0102 (status + metadata + thumbs), Phase 4 (editor blocks, slash menu, keymap), Phase 2 (sidebar).
- TASK-0105 exercises this end to end.
Definition of Done
- [x] 3 upload paths (drop/paste/button) with progress + cancel, all to the

- TASK-0104 (file management) recorded decisions & conflicts:
- ⚠️ Spec-vs-architecture conflict:
packages/permissionsvsapps/api/src/lib/authorize.ts(rule 29 — recorded, followed the architecture, did not guess). The spec's Backend Changes say to extendpackages/permissionswithresourceType: "file"resolution (inherit chain file→block→page→team→workspace; explicit NONE deny). BUTdocs/architecture/search-architecture.md§Permissions states "The API does not use@factory/permissions; its single permission walk isapps/api/src/lib/authorize.ts::effectiveLevel", andpackages/permissionsis imported ONLY byapps/realtime(not the API). So the file-permission resolution (the most-specific NAMED resource + the explicit-NONEhard-deny walk) was implemented inauthorize.ts::effectiveLevel— the LIVE path every read goes through — not in a package the API never calls. TheNONE-deny walk checks the named-scope chain (file > record > page > database > teamspace); aNONEgrant on the file OR its named page is a hard stop that overrides any role baseline / less-specific grant. NoNONEgrants exist in prod pre-0104 (share levels are VIEW/COMMENT/EDIT), so this is a pure addition. Unit-pinned inapps/api/src/common/__tests__/authorize.test.ts(5 new NONE-deny cases). Recorded here rather than silently picking one package. - Quota counter model = EVENT counter, not a live sum (recorded). The
spec's own inline list ("complete +size, purge/replace-old-versions
−size, restore no change") is the event counter, and the acceptance test
asserts the column after
complete("counter accurate, not a sum — assert the column, not a recompute"). SoWorkspace.usageStorageBytes(BigInt) is maintained transactionally byadjustWorkspaceStorageBytes(modules/files/quota.ts):complete +size, prune −Σ,purge −Σ,restore ±0. A naive ledger recompute (Σ File.sizeorΣ FileVersion.size) is KEPT (recomputeWorkspaceStorageBytes/authoritativeStoredBytes) for the reconciler ONLY, because it undercounts retainedVIRUS_DETECTED/FAILEDbytes (noFileVersionrow) and misses in-flightSCANNINGbytes. The pre-check reads the counter O(1); only the DENIAL boundary re-checks the sum, and only a stale-LOW counter (drift > 1 GB) is corrected before a413(spec §Security: "re-check the sum if the counter looks stale > 1 GB drift"). - Backfill approximation (recorded). The pre-existing migration
20260929000000_workspace_usage_storage_bytesbackfilledusageStorageBytesfromΣ File.size(one file row), not theFileVersionledger — so a workspace whose only bytes are in retained old versions starts with a slightly LOW counter. This is safe (LOW = conservative = over-denies at worst) and is repaired by the reconciler; the spec's acceptance test asserts the counter AFTER a freshcomplete, so the backfill value does not affect it. No NEW migration was needed for the column (it already exists); TASK-0104 adds no schema change. effectiveStorageQuota/PLAN_USAGE_LIMITSmoved toquota.ts. They lived inuploads.service.ts; moved tomodules/files/quota.ts(the quota module) so the workspace usage endpoint (GET /workspaces/:id/usage) shares the EXACT same plan quota for the storage bar.uploads.servicere-exports them (the existingfiles-limits.test.tsimport is unchanged).versions()leaked the rawkey— fixed (spec §Security). The first cut ofGET /files/:id/versionsreturnedkey: v.key; the spec is explicit the raw object key NEVER leaves the server (signed URLs only). Removed; the version list now returnsversion/size/created/current.- Orphan-sweep core is root-importable.
apps/worker/src/processors/ files/cleanup.tsimports only@prisma/client+@factory/storage(no worker logger — logging is injected) so the ROOTscripts/storage-orphans.ts(pnpm storage:orphans) can import the SAMErunOrphanSweep(the root has nopinohoist;@prisma/client+@factory/storageDO resolve there via tsx). The dailymaintenance:orphansrepeatable and the CLI are two entry points to the one core; the CLI is dry-run by default (--purgeto delete,--file <id>to scope — the "reports 0" acceptance check). @factory/storage::S3Client.listObjectsgained acontinuationTokenparam (3rd arg, optional) so a fullfiles/sweep pages past the 1000-key cap (the orphan sweeper needs it; the parser already returnednextToken, the method just never forwarded it). Backward-compatible (existing 2-arg callers unaffected).- Frontend (4 surfaces) done + live-verified; one pre-existing type drift
noted (not fixed, out of scope). The four TASK-0104 UI surfaces
(file-block context menu Replace/Rename/Permissions/Delete; FileBrowser
Trash tab + per-row Versions menu + "Delete old versions"; the file
permissions dialog; the workspace storage bar in
settings/billing) are implemented inapps/weband live-verified against:4000(bare arrays for trash/versions/permissions;{trashed,deletedAt}/{purged}/{pruned}for the deletes).web tscflat at 210, files unit 23/23. The fork flagged a pre-existing wire/type mismatch: the live workspace object carries the plan asbillingPlan, but the webWorkspacetype declaresplan(the storage bar readsbillingPlan ?? plandefensively). Aligning theWorkspacetype tobillingPlanis a follow-up (not done here — out of TASK-0104 scope, and other consumers rely onplan). - ⚠️ Spec-vs-architecture conflict: file-level grants are NOT in the
search snapshot (rule 29 — recorded, followed the architecture, did not
guess). Acceptance criterion 7's wording is "re-indexed →
permission-filtered search now includes it for a team member", which
implies a file-level
Permissiongrant changes search visibility. ButbuildFileDoc(apps/worker/.../search-index/builders.ts) computes the file doc's grants viabuildGrantSnapshotover scopes[workspace, teamspace, page]only — file-levelPermissionrows are filtered OUT (grants.tsscopeKeys.has(...); the comment: "file docs inherit the workspace floor with no resource-specific grants"). Search visibility is WORKSPACE-scoped (a member sees a workspace's files); a file-level grant is enforced at READ time (authorize.ts::effectiveLevel), not search time. So a file-levelVIEWgrant to a non-member does NOT make the file appear in the non-member's search (and cannot — the non-member isn't a workspace member). The re-index path (permission.changed→runReindexworkspace resync) IS verified in the integration spec (doc + mirror-row convergence, permission.changedpublished to the event bus), but "search now includes it for the grantee" is asserted only for WORKSPACE-floor visibility, not file-level grants. Recorded rather than special-casing file-level grants into the search snapshot (which would diverge from the architecture's workspace-floor model). - Rename re-index was MISSING — added
file.updated(criterion 8).rename()originally only wrote thefile.renamedaudit and mutated the row; it published NO event, so the search doc's title stayed stale until the 15-min reconcile sweep. Criterion 8 requires the re-index to pick up the new title. Fix:rename()now publishesfile.updated({workspaceId, fileId, name}) after the audit; the worker's search-index processor added acase "file.updated"(re-upsert viabuildFileDoc, same asfile.uploaded/file.finalized).file.updatedis in the shared@factory/shared/eventscatalog and the worker'sEVENTS/SEARCH_INDEX_EVENTS(it is NOT a webhook-catalog event — it is a convergence trigger, not a user-facing notification). Verified: PATCH →file.renamedaudit +file.updatedon the bus + the search mirror doc'stitleconverges to the new name (integration spec, criterion 8). - Purge orphaned the non-primary thumbnail variants — FIXED (caught by
the new integration spec).
purge()deleted the singleFile.thumbnailKey(the 480 variant), but the worker's thumbnails step writes ALL planned artifacts (an image → 160/480/1280 underthumbs/{ws}/{fileId}/{size}/…), so purging a 2-version 1400px PNG left the 160 + 1280 objects behind — a genuine orphan thestorage:orphanssweep would then flag. The spec says purge removes "object + all version objects + thumbnails + DB rows" (the whole set). Fix: adeletePrefixhelper inapps/api/src/modules/files/s3.ts(list-then-delete, paginated past the 1000-key cap, best-effort per key) is used to sweep the entirethumbs/{ws}/{fileId}/prefix. The integration spec (criterion 6) now assertsthumbs/{ws}/{fileId}/is empty after purge andrunOrphanSweep({fileId})reports 0. This is a real bug, not a test artifact — the version object keys were already deleted correctly; only the multi-variant thumbnail set leaked.
- ⚠️ Spec-vs-architecture conflict:
TASK-0104
Title
File management: versions, permission-gated download + audit, trash/restore/purge, storage quota, file-level permission overrides
Objective
Complete file lifecycle management: re-uploading creates a FileVersion,
downloads are permission-checked and audited (denials audited too), delete
goes to trash with restore/purge (including storage cleanup on purge),
storage quota is enforced per workspace from usageLimits, and file-level
permission overrides work on top of page/block inheritance.
Background
FileVersion (file_versions: fileId, version, key, size,
createdBy, unique [fileId, version]) and File.deletedAt (soft delete)
exist. Permission model: granteeType (user/team/role), resourceType
incl. "file", resourceId, level (NONE/VIEW/COMMENT/EDIT/FULL_ACCESS),
inheritsFrom. Workspace.usageLimits Json (TASK-0101 defines the shape).
Quota accounting: sum of File.size over non-deleted files per workspace
(materialized counter usageStorageBytes on Workspace? — decision:
compute via a counter column updated in the file service to avoid
O(n) sums; add the column in this task's migration). Audit: AuditLog
model exists; required events file.uploaded, file.downloaded,
file.download_denied, file.deleted, file.restored, file.purged,
file.version_created, file.permission_changed.
Requirements
- Versions: re-uploading to an existing file (
replaceFileIdin the upload session, UI: "Replace file" on a file block/card) stores a new object key and insertsFileVersion { version: n+1 }; the old object is retained (no delete) until purge or an explicit "Delete old versions" action (admin, confirms, deletes orphan keys + rows, keeps the current).File.key/size/metadata/thumbnailalways point at the current version;GET /api/v1/files/:id/versionslists them;POST /api/v1/files/:id/rollback(FULL_ACCESS) makes version n current (key swap, no new object). - Download permission + audit:
GET /api/v1/files/:id/download+.../preview+GET /api/v1/files/:id(metadata) resolve the effective level: explicitPermissionrows forresourceType: "file"(most specific wins; NONE level = explicit deny overrides inheritance) → else inherit from the containing block → page (Phase-3 permission service). Minimum VIEW. On success: auditfile.downloaded(actor, workspace, resourceId, ip). On denial: 403 + auditfile.download_denied(with the attempted actor + path). VIRUS_DETECTED files: 403FILE_QUARANTINED+ audit (no signed URL ever issued). - Trash:
DELETE /api/v1/files/:id(EDIT on the file's page or file grant with ≥ EDIT) setsdeletedAt(soft), keeps the object + versions, removes from search index (emitfile.deletedevent — add to@factory/shared/events), auditfile.deleted. Restore (POST /api/v1/files/:id/restore, same permissions) clearsdeletedAt, re-indexes (re-emitfile.uploadedwithrestored: trueflag), auditfile.restored. Purge (FULL_ACCESS or workspace Admin/Owner; from the workspace trash view, Phase 3): deletes object + all version objects + thumbnails + DB rows (File,FileVersion), auditfile.purged(irreversible — confirm dialog in UI). - Storage quota: effective quota =
usageLimits.storageQuota(plan default if unset). Accounting:Workspace.usageStorageBytes(BigInt column, migration) updated transactionally on: complete (+size), purge/replace-old-versions (−size), restore (no change). Upload session pre-check (TASK-0101) usesusageStorageBytes(not a live sum). When over:QUOTA_EXCEEDEDnaming used/limit. Dashboard: workspace settings shows "Storage: X / Y used" bar (settings UI hook from Phase 2). - File-level permission UI: file block context menu + file browser row →
"Permissions…" dialog listing explicit grants (user/team/role × level)
with add/remove (same component pattern as page sharing, Phase 3),
"Inherit from page" shown as the default state; changes →
Permissionrows +permission.changedevent (re-index) + auditfile.permission_changed. - Listing/management endpoints:
PATCH /api/v1/files/:id(rename:nameonly; size/mime immutable), trash listGET /api/v1/files/trash(workspace files,deletedAtset), purgeDELETE /api/v1/files/:id?purge=true(guarded).
Acceptance Criteria
- Integration: re-upload to file F (v1 READY) → v2 created
(
FileVersion.version = 2), both keys exist in the bucket, F points at v2;POST /rollback→ F points at v1's key;GET /versionslists both. - Integration: user U (no access) downloads F → 403 + audit row
file.download_denied(actorId = U, ip present); member download → 200/302- audit
file.downloaded.
- audit
- Integration: explicit file grant NONE to a page-editor → 403 (explicit deny beats inherited EDIT); grant VIEW to a non-member → 200 (grant grants).
- Trash: delete →
deletedAtset, file absent fromGET /filesand search (index doc gone within 30 s), restore → back in list + search, purge → rows + objects + thumbs gone (bucket key list empty), audit trail has deleted → restored → purged in order. - Quota: workspace at quota 1 GB with 950 MB used: a 60 MB upload session →
413
QUOTA_EXCEEDED; a 40 MB session → 201 andusageStorageBytes= 990 MB after complete (counter accurate, not a sum — assert the column, not a recompute). - Purging a 3-version file removes exactly 3 object keys + thumb keys
(assert via S3 list), zero orphan keys (a
pnpm storage:orphanscheck script reports 0). - Permissions dialog: adding a team VIEW grant on a file is visible in the dialog, re-indexed (permission-filtered search now includes it for a team member), audited.
- Rename:
PATCHchanges only the name;metadata, size, mimeTypeunchanged; the search doc title updates (re-index).
Backend Changes
apps/api/src/modules/files/:files.service.ts(version ops, trash/restore/purge, quota accounting, permission resolution viapackages/permissions), controllers:POST /rollback,GET /versions,DELETE /:id(trash),POST /:id/restore,DELETE /:id?purge=true,GET /files/trash,PATCH /:id,GET/PUT /files/:id/permissions.packages/permissions: extend resource resolution withresourceType: "file"(inherit chain file → block → page → team → workspace; explicit NONE deny semantics) — unit-tested.apps/worker/src/processors/files/cleanup.ts: orphan-key sweeper (daily job: keys underfiles/{ws}/with noFile/FileVersionrow → delete after 7-day grace + auditfile.orphan_purged).@factory/shared/events: addfile.deleted.scripts/storage-orphans.ts(pnpm storage:orphans).
Frontend Changes
- File block context menu: Replace, Rename, Permissions…, Delete (trash).
- File browser: trash tab (with Restore/Purge + confirm dialog for purge), version menu on rows, "Delete old versions" admin action (confirm).
- Permissions dialog component (shared with pages, Phase 3 component).
- Workspace settings: storage usage bar (X/Y + % + plan label).
Database Changes
- Migration:
Workspace.usageStorageBytes BigInt @default(0)+ backfill (sum of non-deleted file sizes per workspace, batched). - Indexes:
@@index([workspaceId, deletedAt])on files (present);FileVersionunique (present).
API Changes
- New/changed endpoints per Requirements §1–§6; all documented in
docs/api/files.mdwith the audit-event table.
Tests
- Unit: permission resolution matrix (inherit/explicit/NONE-deny across file→block→page), quota counter arithmetic (complete/purge/rollback), version swap logic.
- Integration: all Acceptance Criteria (versions, audit, trash round-trip, quota boundary, orphan check, permission grant + re-index).
Security
- Every download/preview/metadata read permission-checked (server-side); denials audited (who tried, which file, from where).
- Purge is the only path that deletes objects; it requires FULL_ACCESS or Admin/Owner and a confirm; the sweeper has a 7-day grace and its own audit event.
- Quota counter updated transactionally with the file row (no drift window); counter is advisory-fast + authoritative at session time (re-check the sum if the counter looks stale > 1 GB drift — defensive).
- Explicit deny (NONE) is a hard stop — cannot be bypassed by role.

TASK-0105
Title
Files E2E + security suite (500 MB boundary, EICAR, MIME spoof, path traversal, unauthorized access)
Objective
Prove the Phase-9 exit criteria and the file security model with one dedicated E2E + security test suite: happy-path previews for every supported kind, the 500 MB / 50 MB plan boundaries, malware blocking, MIME spoofing rejection, filename sanitization, and unauthorized-access denial with audit.
Background
Tasks 0101–0104 ship storage, pipeline, UI, and management. The CI Docker
profile includes ClamAV (TASK-0102) and MinIO (Phase 0). tests/e2e/ is
Playwright; tests/integration/ is API+Postgres+Redis+OpenSearch+storage.
Fixture files live in tests/fixtures/files/. The 500 MB upload must use
multipart-friendly streaming (presigned PUT of a sparse file) and must run
in CI without exceeding disk limits (sparse file on the runner).
Requirements
- E2E happy path (
tests/e2e/files.spec.ts): (a) upload an image (PNG) → renders inline + a thumbnail appears in the file picker grid; (b) paste a screenshot (clipboard) → renders; (c) upload a PDF → preview iframe + first-page render visible; (d) upload an MP4 → native player plays; (e) upload a DOCX → download card with icon + size, Download delivers the file. - Size boundaries: (a) free plan (50 MB/file): a 60 MB upload is rejected
at session creation with 413
FILE_TOO_LARGE(clear message naming the 50 MB limit) — E2E (toast) + integration (raw API); (b) paid plan (500 MB/file): a 500 MB file (sparse, streaming) uploads via presigned PUT and reaches READY (E2E with a 500 MB fixture generated on the fly; assert status READY + size exact; this is the ROADMAP exit criterion "upload of 500 MB file works (multipart)"). - Malware: EICAR test file (standard anti-virus test string) uploaded on
the ClamAV CI profile → status VIRUS_DETECTED; the file block renders
the blocked card;
GET /download→ 403FILE_QUARANTINED; audit rowfile.download_denied(orfile.quarantine_denied) present; the file is absent from search results; no thumbnail keys in the bucket. - MIME spoof: a text file containing plain ASCII, renamed
evil.png(simulating an exiftool-style rename without re-encoding) → session accepted (name-based), PUT completes, completeruns magic-byte sniff → 422MIME_MISMATCH, status FAILED, object deleted from the bucket, block shows the failure state (integration + E2E for the block state). - Path traversal: filenames
../../../etc/cron.d/x,..\\..\\win.ini,a/../../b.png,bad\x00name.png→ stored object keys are alwaysfiles/{ws}/{fileId}/{v}/<sanitized>(assert the actual S3 key via the storage client in integration: no.., no leading/, no NUL); the displayed name is sanitized but recognizable; no request ever produces a key outside the workspace prefix (property-style test over 200 generated malicious names). - Unauthorized access matrix (integration, 6 cases): non-member GET metadata → 403 + audit; non-member download → 403 + audit; member of a workspace but no page access → 403 on a file inside that page; explicit file VIEW grant to a non-member → 200; grant revoked → 403 (and search re-indexed out); signed URL after 900 s → 403 from storage; signed URL for file A used against file B's key → 403 (key binding, TASK-0101).
- Reporting: the suite writes a summary to
tests/e2e/reports/files-security.json(scenarios, pass/fail, timing) for CI artifacts; any failure blocks the phase (exit criterion).
Acceptance Criteria
- All 5 happy-path E2E scenarios pass (image+paster, PDF preview, video plays, document card, picker thumbnail).
- 500 MB sparse upload on the paid plan reaches READY with exact size; 60 MB on the free plan is rejected at session creation with the clear error (both E2E- and API-level).
- EICAR → VIRUS_DETECTED end to end: blocked card in UI, 403 download + audit, no index doc, no thumbnails (asserted across API, bucket, and search).
- MIME-spoofed PNG (text bytes) →
MIME_MISMATCH, object deleted, block failure state rendered; a legitimately re-encoded PNG (exiftool-style metadata-only change) still passes (positive control). - 200 generated traversal/malicious names → 100 % keys within
files/{workspaceId}/prefix, zero../NUL/absolute paths in keys (integration property test). - All 6 unauthorized-access cases return 403 (or 200 for the grant case) with the correct audit rows; expired + cross-key signed URLs rejected.
- Suite report
files-security.jsongenerated and attached to CI; suite runtime < 15 min (500 MB upload included).
Backend Changes
- None functional (asserts existing behavior); small test hooks only:
expose
GET /api/v1/debug/files/:id/keybehindNODE_ENV=testfor key assertions (or inject the storage client into integration tests — preferred, no debug endpoint; choose the test-client injection approach). - CI Docker compose: ClamAV service enabled in the E2E profile; sparse
file helper
tests/fixtures/files/make-sparse.ts(500 MB, no disk cost).
Frontend Changes
- None (asserts TASK-0103 surfaces).
Database Changes
- None.
API Changes
- None.
Tests
- E2E:
tests/e2e/files.spec.ts— happy paths (5), 60 MB free-plan rejection, EICAR blocked card, spoof failure card, unauthorized block state (stale URL → "No access" card). - Integration:
tests/integration/files-security.spec.ts— 500 MB upload (paid), MIME mismatch + bucket deletion, traversal property test (200 names), unauthorized matrix (6), signed URL expiry + key binding, audit-row assertions per case. - Performance guard: suite timing budget enforced (fail if > 15 min to catch CI regression).
Security
- The suite itself must not leak: fixtures are synthetic; EICAR is the standard test string (safe, non-malicious); no real user data.
- Audit assertions verify the ACTOR is recorded correctly (no "system" misattribution) and IP is present.
- Any scenario that would require bypassing auth is forbidden by design — the suite only uses legitimate auth + API surfaces (AGENTS.md rule 7).
Documentation
docs/security/files.md(new): threat model for files (spoof, traversal, malware, quota abuse, URL leakage), mitigations mapped to tasks, how to re-run the suite, EICAR explanation.docs/development/testing.mdaddendum: fixture conventions (sparse files, EICAR), ClamAV in CI.
Dependencies
- TASKS-0101…0104 complete (everything is asserted, nothing is built).
- CI: ClamAV in the E2E Docker profile; sufficient runner disk for the 500 MB object in MinIO.
Definition of Done
- [x] 5 preview happy paths E2E-green (image, paste, PDF, video, document).
- [x] 500 MB paid upload works; 50 MB free boundary rejected clearly (the ROADMAP exit criterion).
- [x] EICAR blocked end to end (UI + API + audit + no index/thumbs).
- [x] MIME spoof rejected + object deleted; positive control passes.
- [x] Traversal property test: 200 names, zero unsafe keys.
- [x] Unauthorized matrix: correct 403/200 + audit on all 6 cases.
- [ ] Report artifact generated; suite < 15 min; CI green.
Verified locally: artifact committed (
tests/e2e/reports/ files-security.json, 6 scenarios, 45.4 s, within the 15-min budget) and both suites green (integration 6/6, e2e 15/15). CI re-run is pending — the push of these commits is blocked by a pre-existing>100 MB.pnpm-storeblob in the 09-26..09-28 history (push-blocker note below), so the CI leg is not yet independently confirmed. - [x]

tests/e2e/specs/files.spec.ts, 15/15 green onchromium-desktopagainst the live stack (web :3100, API :4000, worker :4003, MinIO, OpenSearch, ClamAV;RATE_LIMIT_*raised as CI does): the 5 happy paths (image, paste, PDF, video, document) + the TASK-0105 additions — clipboard paste → new file block, DOCX download card delivering byte-exact bytes, 60 MB free-plan file → "File too large" toast with no session opened, non-member block state (bounced, nofile-cardin DOM), picker thumbnail (real<img>). Thechromium-mobile(webkit) project cannot run on this Ubuntu 26.04 host (Playwright 1.49 webkit links libvpx9/libicu74/libx264-164; host has libvpx12/libicu78/libx264-165) — it is unchanged and runs in CI (ubuntu-latest +install-deps); local runs target the primary surface, matching prior TASK-0103 runs. - Report artifact —
tests/e2e/reports/files-security.json(committed as reference evidence; CI re-generates it): 6 scenarios,totalDurationMs45,354,withinBudgettrue. - CI — not verifiable from this host (the repo's CI runs on
ubuntu-latest; the webkit
chromium-mobileproject in particular cannot run here — see above). Local verification: integration 6/6 + e2e 15/15 onchromium-desktopagainst the live host stack, both green after the final fixes. The commits are local (main, 3 commits on top of the 09-30 sync); the push — and therefore the authoritative CI re-run — is pending behind the push-blocker below. - Push blocker (pre-existing repo hygiene, NOT from this task). The
84-commit push range (
origin/maindb701ab3 (09-25 sync) → HEAD) contains the pnpm store cache at the repo root —Coding/notion/.pnpm-store(2.4 GB, ~44k tracked blobs, 7 of them >100 MB) — committed by the 09-26 / 09-28sync:commits andchore(deps): TASK-0100 S4 prepand still tracked in HEAD (.dsh-pnpm-env.sheven pointsNPM_CONFIG_STORE_DIRat it; the root.gitignorehas no entry). GitHub's pre-receive hook (GH001 large-file detection, 100 MB per-blob limit) rejects the whole push. None of TASK-0105's commits touch it (largest file added here: the 92 KBhero-1400.pngfixture). Decision recorded 2026-09-30 (user choice): leave TASK-0105's commits local rather than rewrite the 84-commit history or force-push. Follow-up to unblock all unpushed work:git rm -r --cached Coding/notion/.pnpm-store+ root.gitignoreentry (fix-forward), thengit filter-repo --path Coding/notion/.pnpm-store --invert-pathsover the unpushed range + force-push, coordinated with the sync tooling that owns thosesync:commits. - Regression test for a product bug found by the new E2E (rule 9): the
paste scenario caught that
blockApi.insert(apps/web/src/lib/api.ts) POSTed toPOST /api/v1/blocks— a route that does not exist (404Cannot POST /api/v1/blocks) — instead of the realPOST /pages/:pageId/blocks(BlocksController.create). Every client-side new-block path was broken by this: paste, drag-drop insert, the file picker's "Insert into the page", the context-menu paste, and the editor'suse-blocksinsert queue (in-place file-block updates were unaffected —blockApi.updateused the correctPATCH /blocks/:id). The file itself uploaded fine (READY); the block just never appeared. Fixed to`/pages/${input.pageId}/blocks`(all callers passpageId— it is required inInsertBlockInput); the paste e2e test is the regression guard. Verified at runtime by the green paste/DOCX/picker e2e tests. (The web app's fulltscwas already red with 210 pre-existing errors before this change —packages/ui, share/comments/database pages — documented above; the changed filelib/api.tscontributes none.)
TASK-0105 (files E2E + security suite) — recorded decisions & conflicts (rule 29)
- ⚠️ Spec-vs-implementation conflict: MIME-spoof status code (Req 4). The
Requirement 4 line says
completereturns 422MIME_MISMATCH; the implemented, architecture-consistent contract (and the TASK-0103/0104 suites, anddocs/api/files.md§9) is 415MIME_MISMATCH(Unsupported Media Type — the media itself is wrong, not the request envelope). Followed the architecture/implementation (415); recorded here rather than guessing. The security property is identical either way: rowFAILED,metadata.pipeline.error="mime_mismatch", object deleted from the bucket, auditfile.mime_mismatch. - Decimal-vs-binary "500 MB" boundary (Req 2b). The ROADMAP/suite speak
of a "500 MB" file; the plan limit is 500 MiB (
500 * 1024 * 1024 = 524,288,000 B,quota.tsPLAN_USAGE_LIMITS.paid). The sparse fixture is 500,000,000 decimal bytes (make-sparse.tsSPARSE_500_MB) — below the paid limit (so it reaches READY on paid) and above the free 50 MiB (so it would 413 on free). This is the conservative choice: the exact boundary is asserted in the TASK-0104 quota suite (a file at the 50 MiB limit passes, one over 413s); the 500 MB case here proves the large-object round trip with headroom. - Traversal invariant is SEGMENT-level (Req 5). The spec says "no
.." in keys. A name that merely contains the dot-dot substring (a..b.png) is a single harmless object name in a flat S3 namespace — the dangerous case is..as a path component, whichsanitizeFilenameremoves. The property test therefore asserts (on all 200 generated hostile names): workspace prefix, no NUL, no backslash, no leading/, and no../.or control-character segment — not a raw substring ban, which would be false-positive. Real uploads additionally prove the object physically lands at the sanitized key and the display name stays the original. - Unauthorized matrix: page-level
NONEgrant is seeded via the seam (Req 6c). Case (c) needs "a member with no access to the page." Phase 9 exposes file-level grants only (PUT /files/:id/permissions); page-level grants have no public API route (the grantee model exists, but only file grants are surfaced). The suite seeds the pageNONEgrant through the same internal-seam / direct-DB convention used elsewhere for "state the API cannot create" — the API's enforcement of that row (the hard-deny that beats the member's inherited EDIT) is what is under test; nothing bypasses authorization (rule 7). (If a page-grant API is added later, the seam write can be replaced with a realPUT.) - File-level grants are read-time only (Req 6e search). The matrix's revoke case asserts the download 403s after revoke, but the search doc is not removed: search visibility is workspace/page-scoped by design (carried from TASK-0104's recorded note) — file-level grants are enforced at read time, not by re-indexing. A revoked member can still find the file and then get 403 on open.
- EICAR scanReport location (Req 3). The virus finalize writes
scanReportto the top-levelFile.scanReportcolumn (not undermetadata.pipeline) and setsmetadata.pipeline.error="virus_detected". S5 asserts the top-level column ({engine:"clamav", infected:true, signature:"Eicar-Test-Signature FOUND"}). The scan is driven in-process withenv.CLAMD_SOCKET= the host ClamAV unix socket (zCONTSCAN, the worker's production path; TCP:3310is broken in the pinned image). - 500 MB / EICAR / signed-URL assertions are INTEGRATION, not E2E (Req 2b, 3, 6). The spec's E2E list (Req 1) covers the 5 happy paths + the 60 MB free-plan toast (a client-side pre-check — no 60 MB bytes are sent in the browser; the authoritative server 413 is asserted in integration, as TASK-0103 already does for the oversized case). The 500 MB upload, EICAR quarantine (no thumbs/index/403-download), and the signed-URL expiry/key-binding matrix need the real pipeline + storage + ClamAV, which the integration harness drives in-process against the same host stack the e2e uses. E2E adds the paste, DOCX byte-round-trip, 60 MB toast, unauthorized block state, and picker thumbnail (Req 1 + the DoD happy paths).
- E2E mobile surface = webkit, which does not run on the local 26.04 host.
The Playwright config's
chromium-mobileproject usesdevices["iPhone 13"], whosedefaultBrowserTypeis webkit. The pinned Playwright 1.49 webkit build (2104) linkslibvpx9/libicu74/libx264-164— none of which exist on this Ubuntu 26.04 dev host (it shipslibvpx12/libicu78/libx264-165, a different SONAME). CI (ubuntu-latest, withplaywright install-deps) runs it fine. Local runs therefore target the primary surface:--project=chromium-desktop(matching prior local runs). This is a host toolchain limitation, not a code/test defect — the webkit project is unchanged and still green in CI. - Search-presence assertions force an index refresh (test determinism, rule
25). search-index's bulk
indexDocsis sent withoutrefresh, and thefactory-content-v1index usesrefresh_interval: 1s, so a_mget(docExists) immediately after indexing races the refresh. The suite'ssearchEvent()(and the post-runReindexrevoke assertion) now callsearch.refresh()before asserting — the client's documented escape hatch for "tests / immediate lag" (SearchClient.refresh). No product behavior changes; the search contract stays eventual (reconcile backstops), this only makes the test's existence check deterministic. - Host finding (not a defect): disk flood-stage blocked OpenSearch writes.
Repeated S1 runs persist the full 500 MB of the sparse file as real
bytes in the MinIO
factory-filesbucket (sparse-on-disk, not sparse-in-storage), which filled the host root disk past OpenSearch's flood-stage watermark (~95%). OpenSearch then setfactory-content-v1toread_only_allow_deleteand every file-index bulk returned 429 cluster_block_exception. This surfaced: (a) the suite's S4 "doc present in search" assertion depends on host disk headroom (CI's fresh OpenSearch is unaffected); remediation is to purge the stalefiles/**/big.bin500 MB objects and clear the transient block (PUT factory-content-v1/_settings{"index.blocks.read_only_allow_delete":null}); and (b) a pre-existing observability gap in search-index (TASK-0100 territory, recorded here, NOT fixed by this task):indexDocscounts per-itemstatus < 300and returns a count without throwing, and thefile.uploadedhandler logs "file indexed" unconditionally — so a read-only OpenSearch silently no-ops file indexing (only the 15-min contentHash reconcile backstops it, and it would hit the same 429). Follow-up candidate: surfaceindexed < docs.lengthas a warn/retry in the handler.
